This is the template we use to formalize your booking. After you submit a request on our site, we send this as a pre-filled PDF tied to your Square invoice. You sign it digitally or by hand, and we countersign.

5. Cancellation & weather
Governed by our Cancellation & Weather Policy, incorporated by reference. Summary: 25% deposit is non-refundable; reschedule windows apply; we work with Client on weather holds; we reserve the right to cancel for safety with full refund offered.
6. Client responsibilities
Provide accurate event details, venue address, and point-of-contact at least 7 days prior.
Provide a level, accessible setup space of at least 15' × 25' for on-site packages.
Inform us of any guest allergy/dietary concerns at least 7 days prior.
7. Caterer responsibilities
Arrive with trailer/supplies at least 30 minutes before contracted service start.
Provide all drinks, cups, lids, straws, garnishes, and staffing per selected package.
Hold active Illinois Mobile Food Establishment license, McLean County health permit, and commercial general liability insurance (copies available on request).
Food handler-certified staff.
Clean up service area upon departure.
8. Liability
Client will ensure all event attendees read and follow any safety guidance from Caterer. Caterer's total liability for any claim under this contract is limited to the amount paid by Client. Caterer is not liable for indirect, incidental, or consequential damages, or for outcomes attributable to third-party venues or vendors.
9. Allergies & ingredients
Our drinks may contain: dairy, soy, coconut, caffeine (in Lotus/Red Bull add-ons), tree nuts, sugar, and common flavoring syrups. We do not operate an allergen-free kitchen. Guests with severe allergies should verify ingredients with staff before ordering.
10. Intellectual property & photography
Caterer may photograph the event setup, drinks, and anonymous crowd shots for marketing use. If Client wishes to opt out, check here: ☐ and initial: _______.
11. Dispute resolution
Any dispute under this agreement shall be resolved by good-faith negotiation first, then by mediation in McLean County, Illinois, and finally (if necessary) by a court of competent jurisdiction in McLean County, Illinois. Illinois law governs.
12. Entire agreement
This contract, together with the Square invoice, Terms of Service, Privacy Policy, and Cancellation & Weather Policy, constitutes the entire agreement. Any changes must be in writing and signed by both parties.

Legal note for Frost/Mike: This is a working template based on industry-standard mobile catering contracts. Before using it in production for high-value bookings (weddings > $1,500), have an Illinois-licensed attorney review it. Typical review cost is $250-500 flat fee and is well worth it for legal enforceability.
